Patient Responsibilities
Patients receiving services from EverCare Medical Supply are responsible for:
- Providing accurate and complete information related to medical condition, insurance coverage, and physician orders.
- Using equipment according to provided instructions and safety guidelines.
- Notifying EverCare Medical Supply of equipment malfunction, damage, or service needs promptly.
- Maintaining equipment in a safe and appropriate environment.
- Informing EverCare Medical Supply of any changes in address, phone number, physician, or insurance coverage.
- Meeting agreed financial obligations for equipment or services not covered by insurance.
- Returning rental equipment when services are discontinued or no longer medically necessary.
- Allowing reasonable access for equipment delivery, setup, maintenance, or retrieval when applicable.
